Preparing for your first appointment

For your convenience, you may download our New Patient Forms, print and fill them out prior to your first visit. Please bring the forms with you to your first appointment. The forms may also be obtained at the office. We can mail you a copy ahead of your appointment, or you can arrive 15 minutes early prior to your first dental visit.

However, we accept most dental plans and we will be happy to submit your claim on your behalf. To utilize this service, we ask that you bring a copy of your dental insurance card or all necessary information. (Subscriber name, birth date, identification number, group number, name and mailing address of your insurance company.)
